Does anyone have or know where to find a free database of US Cities and States? We're willing to pay a reasonable fee, but we were able to find a free Canadian one a year or so ago when we needed one.

With a little searching, you can find web pages that list all the USA cities. Then save those pages to your computer, create and run a small perl script locally on your computer (requires installation of Active Perl on Windows machines or Perl on Linux) or two to extract the information. That's what I'd do if I wanted that info.
Hope that gives you ideas

On maxmind you can download world cities, so just use a query to query USA only and save the results. Free and simple. Can be done with php/asp and loads of other languages, or can be simply done in Excell too...
